How to become a new patient

Hopefully, this page will guide you through the process of becoming a new patient at Keystone Chiropractic Neurology. It is our intention to make your transition as smooth and seamless as possible. We participate only in Highmark Blue Shield plans and co-pays apply. For all other plans, payment is due in full before services are rendered.

Please note: not everyone is accepted to care at our clinic. Because of high demand, we can only accept those who are most qualified and are most dedicated to changing their lives. At times, our waiting list can be lengthy, so apply early to ensure that your wait time is not extended. 

Step One: Request your New Patient Paperwork.

You can do this by calling our office at 717-697-0589, or sending us an email from the Contact Us page. To reduce paper waste, our preferred method of sending paperwork is via email. Please indicate if you require paperwork to be sent through the mail. Please fill out the paperwork completely and thoroughly. Dr. Sullivan wants to get the most complete and accurate picture of your situation possible. If it is apparent that sections have not been filled out, your request will be denied. 

Step Two: Fax your completed paperwork to us, along with a copy of your insurance card.  If you are not the primary policy holder, please indicate who is and include his/her date of birth. This will help us determine what benefits or coverage you might have. If you do not include a copy of your card, we will assume that you do not have insurance and bill charge you accordingly.

Step Three: Call our office to schedule your initial visit.  After you have faxed us your paperwork, you may call to schedule your exam and consultation. As noted above, we participate only in Highmark Blue Shield and co-pays/coinsurance/deductibles will apply. If you have a different policy, payment is due in full before services are performed. The fee for our initial visit is $190.  It is our policy to charge $75 for cancelling without providing 24 hours notice of your initial appointment.

What happens next?

When you meet with Dr. Sullivan, he will ask you questions to clarify your paperwork. He will address your concerns and situation. He will perform a comprehensive neurological examination to determine where your symptoms may be coming from. Dr. Sullivan might elect to start some form of therapy with you at that point, in order to see how your symptoms and/or function responds to care. After your exam, you will be released and scheduled for a report of findings.

On a separate appointment, you will meet Dr. Sullivan for a discussion about your case. Dr. Sullivan will inform you if he is able to accept your case. If he is unable to accept your case, you may be referred to another office or practitioner.

If however, you have been accepted into our practice, you will be given a plan of care to address all of your concerns. Our care is second to none, and is intended to give you the results you’ve been waiting for. You will be given a cost analysis including insurance coverage/reimbursement and out of pocket costs. Various payment options will also be discussed at this time. Generally, specialized laboratory testing is needed to complete your health picture. This will be combined with in-office treatments to give you the best results.

“Can I think about it?” Some of our care plans incur a significant cost, but it is an important investment in your health and we will work with you to make it as easy as possible. You will never be pressured into making your final decision at that time. However, please keep in mind these key points:

1. If you wait too long, or do not keep in communication with us, you may loose your opportunity. As we are in high demand, when spaces open up, they are quickly filled. Dr. Sullivan spends a great deal of time on all of his patient’s cases, which limits the number of people he can treat in any given period of time.

2. Your care plan will be valid for 30 days from your follow up visit. We reserve the right to revise your care plan after 30 days. For care plans that require a significant investment, we have several payment plans available to assist you in managing your health care expenses.
